If you are an former (non-active) member of the Arizona Department of Corrections AND Retired in GOOD standing from the Department (receiving retirement payments from CORP), OR were medically retired in GOOD standing (receiving medical payments), OR are an Active Law Enforcement Officer in Arizona that attended the Correctional Officer Training Academy (COTA) this application is for you!

This application is for Membership ONLY in the Fraternal Order of Police, no Legal Plan is provided under this option.

Active employees of ADCRR are not eligible to use this option and must use the application here: ACTIVE MEMBERSHIP

Depending on option, Fraternal Order of Police, Lodge 44 Dues are $12.00 per month.

By filling out this application for FOP membership you also agree to pay for any optional benefits that you as a member sign up or opt in for and you also agree to assume all responsibility for payment as an individual of the those benefits.
